Application to Take an Exam

Instructions for submitting applications can be found inside each level of the handbook for candidates.  Applications are no longer submitted to CPC, they are submitted directly to Professional Testing Corporation.  See the handbook at the PTC Web site by using the link above.  For applications for the Practical Examination, contact cpc@aoa.org.  Do not submit practical applications to Professional Testing Corporation.

2012 Written Examination Dates and Deadlines:

• Feb 11-25 deadline Jan 2, 2012 (deadline has been extended to January 21, 2012)
• May 5-19 deadline Mar 24, 2012
• Aug 11-25 deadline June 30, 2012
• Nov 3-17 deadline Sept 22, 2012
• June 29 (**paper and pencil exam at Optometry's Meeting, Chicago, IL only) deadline May 18, 2012

 Complete your application using your name exactly as it appears on the photo ID you will present to PSI on the examination date. Registering for the examination with a name that does not match the presented photo ID may result in denial of admittance at your scheduled examination appointment at PSI and forfeiture of the entire registration fee.

 This application is designed to work best with Adobe® Reader® 9. It is recommended that you update to the latest version before filling out your application.



IMPORTANT NOTE: When submitting your application online, you should receive an automated e-mail response within minutes of submission.
