CE Review Application and Forms

The CPC CE approval requirement is in effect. Beginning with the 2013 certification renewal, paraoptometrics will be required to submit education that has been approved by CPC or COPE only. Effective immediately, CPC CE review is complimentary to AOA state optometric or paraoptometric affiliates only. A late fee will apply to all entities not submitting education for review at least 8 weeks in advance of the presentation.

For your convenience, the CE request for review forms have been formatted so that you can complete them while you are on the AOA Web site. However, they cannot be submitted online so remember to save a copy to your computer or print the form. You may send all the documentation directly in an email with the forms as attachments to CPC at cpc@aoa.org, or mail them to:
AOA-CPC, 243 N Lindbergh Blvd, St. Louis MO 63141.

Make sure to complete all four forms for each course for which you are requesting CE review (Cover Sheet, Course Information, Biographical Data, and Checklist.) Course objective, course descriptions and an outline must be submitted for each course. Submissions lacking documentation will be held until all required documentation is received or will be rejected without review. CPC no longer prints forms for education sponsors. Once education is approved CPC will format a CE template for each approved course and email the templates to the contact listed in the application. Education sponsors may then print the required number of CE forms for their presentation(s).
