CE Review Application and Forms

The CPC CE approval requirement of eighteen hours of continuing education is in effect. Beginning with the 2013 certification renewal, paraoptometrics will be required to submit a minimum of nine hours of education that has been approved by CPC; the remaining nine credits may be a combination of education that has received approval from ABO, NCLE, JCAHPO, or COPE.

For your convenience, the CE request for review forms have been formatted so that you can complete and submit them while you are on the AOA Web site.

Make sure to complete one cover sheet for each meeting or presentation, a biographical data  form for each speaker, and a course information form for each course for which you are requesting CE review.  A current resume' or CV for each speaker is also required.  Course objectives, course descriptions, and a detailed and/or timed outline must be submitted for each course. Submissions lacking documentation will be held until all required documentation is received or will be rejected without review. CPC no longer prints forms for education sponsors. Once education is approved CPC will format a CE template for each approved course and email the templates to the contact listed in the application. Education sponsors may then print the required number of CE forms for their presentation(s).
