Order Information
Quantity Ordering: Pamphlets must be ordered in increments of 100 only, i.e., 100, 200, etc. (unless otherwise specified).
Correct Address: Please give complete business street address with all orders. Also, when ordering on behalf of a professional corporation, please include the doctor's name, and member number.
Shipping: Shipping will be charged on all orders. Materials will be shipped via United Parcel Service (UPS) unless otherwise requested. Please allow 2-3 weeks for delivery (3-4 weeks for imprinted items) unless otherwise noted.
Delivery: Delivery dates can only be guaranteed if material is shipped Next Day UPS, 2nd Day or 3rd Day Service. Shipping times for UPS Ground are only approximate. Use our Toll-Free 800 number (1-800-262-2210). Automated telephone available 24 hours a day, 7 days a week. Or, use our Fax number 314-991-4101 to place rush orders. Rush orders on pre-printed materials only can be sent 2nd day, 3rd day or Next Day UPS at customer's expense.
Credit Terms: Terms on invoices are 30 days net. Checks should be payable to American Optometric Association Order Department.
Foreign Orders: All orders from foreign countries (except Canada) must be prepaid. A quote including shipping will be provided. Orders will automatically be insured, at customer's expense; and shipped Air Parcel Post whenever possible. All foreign payments must be made in United States funds.
Return Policy: Pre-printed material may only be returned for credit within 30 days of order date and only if material is in resale condition. Shipping costs will be charged on all returned material.
Price Changes: AOA reserves the right to change any or all prices and to eliminate items without advance notice or substitute revised materials for those orders.
IT IS YOUR RESPONSIBILITY
To check with your state post office for change in printing regulations, also your state association for any legal changes on any form of printing.
SPECIAL STATIONERY ORDERING INSTRUCTIONS
Orders for imprinted materials must be made by mail or by fax (please type or print clearly). Only repeat orders can be placed by telephone. Please have your (Dr.'s) member number ready to place your order.
