Post-Graduate Application Form



As a non-practicing optometrist who is a full-time student in an accredited institution of higher education, you are eligible for complimentary direct membership in the AOA as a postgraduate member. Postgraduate membership continues until the end of the calendar year in which you complete your residency. The postgraduate classification must be renewed annually when appropriate. Instructions for Application for Postgraduate Membership

This application is used to initiate new postgraduate membership or to renew a current postgraduate membership for an additional year. To qualify for postgraduate membership you must meet one of the following qualifications:

  • A non-practicing optometrist who is a full-time student in an accredited institution of higher education.
  • An optometrist who is engaged full-time as a resident or a fellow in a residency program approved by the Accreditation Council on Optometric Education or by a school or college of optometry which is accredited by the Accreditation Council on Optometric Education.
  • An optometrist who is engaged full-time as an intern in an internship program required by state law.

All information on the application must be completed in full. Certification of your full-time status as a student must be made by an appropriate official of the institution and must be noted on the application by their signature. Incomplete applications will not be processed. Postgraduate members have no annual dues obligation. A renewal application is automatically sent by AOA Member Records to those postgraduate members who have indicated that their postgraduate status extends beyond one year. The completed application must be submitted to AOA Member Records, 243 N. Lindbergh Blvd., Floor 1, St. Louis, MO 63141-7881 for processing and approval. Upon approval, you will receive by mail your AOA membership card. If you were previously a student member of the AOA, you will maintain the same ID number. Your AOA ID number is your permanent number and will not change regardless of the changes made to your membership classification and status over the years. Please be aware of your AOA ID number. It is required for member access to the AOA website and free registration at Optometry's Meeting™.
