Associate Member Application - OD Residing and Practicing in a Commonwealth, Territory or Possession of the United States



My current status is:

Direct Associate Member Application

Thank you for your interest in joining the AOA and taking an active role in strengthening optometry.  As an optometrist residing and practicing in a commonwealth, territory, or possession of the United States where there is no affiliated association, you are eligible for direct AOA membership as an associate member.

Effective January 1, 2012, annual dues for an associate membership are $418.50. Annual dues will be pro-rated for the year joined, based on the effective date of membership - the date in which you sign the application. The AOA membership calendar year is January 1 though December 31.

Use the direct associate membership application to initiate a new associate membership, or to reinstate an AOA associate membership that has lapsed. NOTE:  Current associate members should not submit an application for membership renewal. Renewal invoices are issued by AOA in January of each year. Payment of your dues invoice renews your membership for the upcoming year.

Application Instructions

1. Download the application.

2. Complete all application information requested.  Your signature on page two acknowledges acceptance of membership terms. Payment of first year dues must be included with your application.  The dues amount is determined by your effective date of membership (see payment of dues on page 2 of the application).  Dues may be paid by MasterCard, Visa or American Express or by check drawn on a U.S. bank in U.S. dollars.

3. Mail your completed application and dues payment to:

Upon approval, your AOA membership card and certificate will arrive by mail within 4-6 weeks.

Your AOA ID number will be included on your membership card. Your AOA ID number is required for member access to the AOA Web site and to receive the reduced member registration fee at Optometry's Meeting®.
