Membership Application (national and state association/affiliate)

Instructions

• You will first need to select the affiliate you wish to join in conjunction with AOA:
  • The state in which you live or work (including District of Columbia)
  • The Armed Forces Optometric Society if you serve as an optometry officer on active duty in one of the uniformed services, or if you are a full-time employee of the Veterans Administration (VA) or Federal Civil Service (Indian Health Service or Public Health Service).
• Depending on your affiliate selection, you may be redirected to their online join site.  You may need to disable pop-up blocker to continue to the selected site.
• You will need to log in to your existing account with AOA to join on our site. If you were a student or previous member, you most likely have an account.
  • Retrieve your username using the email address you have on file with us
  • Reset your password using the same email address
• If you do not know your email address or no longer have access to it, you can create a new account and we will merge your accounts for you after you join.
